Key Responsibilities

Coordinate and deliver wellbeing-focused programs that support patients in managing their health and daily living
Plan, organize, and facilitate group sessions, workshops, and structured activities
Assess patient needs (physical, emotional, and psychosocial) and contribute to individualized care planning
Collaborate with multidisciplinary healthcare teams to ensure integrated patient support
Provide education and guidance that promotes self-care and independence
Support both in-person and remote/virtual patient engagement initiatives
Maintain accurate documentation and uphold professional clinical standards
Requirements & Qualifications
Registered Nurse qualification with valid professional registration (or equivalent allied health qualification)
Relevant clinical experience in palliative care, community care, or related healthcare settings
Strong communication, interpersonal, and teamwork skills
Ability to assess patient needs and deliver person-centered care
Experience in teaching, mentoring, or facilitating learning is an advantage
Willingness to engage in continuous professional development or postgraduate study
Flexibility to support service needs, including occasional travel within the service area
