Job Description

St. Gertrude’s is

seeking a Minnesota licensed Social Worker to provide social service

assessments, care planning, resident advocacy and relationship building with

patients in our Transitional Care Unit. Candidates must demonstrate strong,

positive, interpersonal skills, flexibility and the ability to work in an

environment with rapid changes.

This is a. 8 temporary position.

Qualifications

• Current social license
• 1 + years’ experience in Skilled Nursing

Facility

• Familiar with federal and state LTC regulations
• CPR certified
• Excellent verbal and written communication

skills

Able to read, write and communicate in English
