Healthcare Account Manager (Facility & Referral Networks)
This role is employed by Phoenix Med Health (MSO), a healthcare management organization supporting medical practices across operations, growth, and technology.
You will work directly on behalf of Active Vital Care (www.activevitalcare.com), a mobile, multi-specialty medical provider delivering in-home care to patients across assisted living facilities, group homes, and private residences.
We’re hiring someone to own and grow our referral network across assisted living facilities, group homes, home health agencies, hospice, and other healthcare partners.
This is a growth-focused role responsible for driving patient volume through strong, consistent relationships.
